Topic: Implementing Whole Person Care to Help the Sickest, Most Marginalized Angelenos Thrive

speaker hong clemensClemens Hong, the Director of Whole Person Care - Los Angeles, will describe implementation of the Whole Person Care Pilot Program in Los Angeles County. This is a 5-year, section 1115 Medicaid Waiver program that brings ~$1.26B to Los Angeles County to build capacity and infrastructure to care for the sickest, most marginalized Medicaid beneficiaries in Los Angeles.

 

 

 

Dr. Hong will:

  • provide an overview of the Whole Person Care programs
  • describe challenges and opportunities faced
  • discuss lessons learned
  • review leadership approaches employed
  • describe plans going forward

About the Speaker:

Clemens Hong MD, MPH is the Director of Whole Person Care at Los Angeles County Department of Health Services. Dr. Hong leads a number of county initiatives to improve community and population health for Los Angeles County residents with a focus on the sickest and most marginalized LA County residents, addressing social and behavioral determinants of health, and integrating community health workers onto health delivery teams. Through the Whole Person Care Pilot, a ~$1.26 billion initiative to provide seamless coordinate care to the sickest and most vulnerable LA County Residents, he is working to strengthen health delivery integration across County and community-based health delivery entities.

The Downtown LA area has a few construction areas. The intersection of Temple Street and Fremont Avenue can be highly congested, please use Figueroa Street and Diamond Street (1 block south of Temple Street) to get to Fremont Avenue where you can safely make a right turn into the parking lot (see star #1 on map below). The maximum parking rate is $9 /day, but please verify with the parking attendant.
